Why blackheads come back after you squeeze them

A blackhead is a pore plugged with oil and dead skin that has oxidized at the surface. Pressing on it from the outside usually pushes part of the plug deeper and ruptures the pore wall — which is what turns a blackhead into an inflamed spot, and sometimes into a mark that outlasts it by months.

Professional extraction works the other way round: the skin is softened first so the plug releases with minimal pressure, and the pore is left clean rather than damaged.

Which treatment fits your skin

  • Deep Cleansing Treatment — $150, 60 minutes. The straightforward option for congestion on the nose, chin and forehead when skin is otherwise calm. A full walkthrough of the deep cleansing facial covers what the session itself involves.
  • Skin Analysis and Deep Cleansing — $175, 60 minutes. Starts with an assessment of what is causing the congestion, then treats it. The better first visit if you have not been treated here before.
  • Clinical Acne Treatment — $210, 75 minutes. For congestion that comes with active breakouts, where extraction alone is not enough.
  • HydraFacial and Acne Treatment — $315, 75 minutes. Combines vacuum-based extraction with hydration for congested skin that is also dehydrated.

What a session involves

  • Softening with steam and an exfoliant so plugs lift rather than tear.
  • Extraction under magnification, worked in short passes so no one area is overworked.
  • Calming to settle the redness that follows extraction — usually gone within a few hours.
  • A home routine built around a chemical exfoliant, because congestion returns without something keeping the pores clear between visits.

How often congestion needs treating

Pores refill. Most clients with persistently congested skin come every four to six weeks, which tracks the skin’s own turnover cycle, and stretch the interval once a home routine is doing more of the work. A six-session package is available for skin that needs a sustained run, at a lower cost per session than booking singly.

When it is not just blackheads

Small bumps that never come to a head and do not extract are usually closed comedones, which respond to consistent exfoliation and an over-the-counter retinol over time rather than to extraction. Some bumps that look like congestion are something else entirely — milia, folliculitis, or a rash that needs a different treatment — and those do not improve with exfoliation. If that is what we see, we will point you to a doctor rather than keep treating it.

Painful, deep, cystic lesions need a dermatologist and usually prescription medication. If your skin looks like that to us, we will say so and refer you on rather than treating it repeatedly.

Frequently asked questions

How much does professional blackhead removal cost in San Francisco?
At Aesthetic OOGII a 60-minute Deep Cleansing Treatment is $150, and Skin Analysis with Deep Cleansing is $175. If congestion comes with active breakouts, the 75-minute Clinical Acne Treatment is $210. A six-session deep cleansing package lowers the per-session cost.
What does professional extraction feel like?
You feel pressure and a brief sting on the more stubborn plugs, particularly around the nose. Skin is softened first so most extractions release easily. It is uncomfortable rather than painful, and the sensation stops when the session does.
Can I come in if I am on Accutane or a retinoid?
Not on isotretinoin (Accutane) — wait at least six months after finishing, since skin is fragile and extraction or exfoliation can injure it. Prescription retinoids are usually fine, but stop applying for about five days before your appointment and tell us what you use so the treatment can be adjusted.
How long does the redness last afterward?
Usually a few hours. Areas that needed more work — typically the nose and chin — can stay pink into the evening. Give your skin the rest of the day bare; makeup the next morning is fine.
Why do my blackheads come back?
Because pores keep producing oil and shedding skin cells. Extraction clears what is there now; it does not change how quickly the pore refills. What extends the result is a chemical exfoliant used consistently at home, which is why a routine is part of every session.
Can I get rid of blackheads at home instead?
You can reduce them with a well-chosen chemical exfoliant. What does not work is squeezing, pore strips, or metal extractor tools used without training — those rupture the pore and cause the marks and scarring that bring people in later.
How often should I come in?
Every four to six weeks while congestion is being brought under control, then less often once the home routine is holding. Skin that is only mildly congested may need nothing more than a seasonal visit.
